Following admission, each patient will be assessed and a subsequent “Pathway of Care” will be drawn up. This Pathway of Care will represent a multidisciplinary outline of care over a period of time to help a patient with a specific condition, or set of symptoms, move progressively through a clinical experience to positive outcomes.
The Pathway of Care is a crucial aspect of a patients stay at Vista Healthcare as it helps to reduce any unnecessary variations in the care of a patient and any outcomes.

Vista Healthcare adopts the Care Programme Approach (CPA) which plays a crucial role in the framework for discharge planning and aftercare.
We provide a systematic assessment of each patient’s health and social care needs.
We then develop a care plan involving staff from the Hospital, the patient and any informal carers. The care plan is put in place to address the assessed needs and is adhered to at all times.
Each patient is assigned a key worker to be the main point of contact and to monitor the delivery of the care plan.
CPA meetings take place for each patient on a regular basis, usually every 6 months. The CPA meeting is a chance for those involved with a patient to review their progress and care plan and make any necessary changes.
